Our Approach to Diabetic Foot Care

Diabetics cannot sense pain and pressure in the foot, but still develop foot ulcers which do not heal due to inadequate blood flow to the foot. We start each case by assessing for neuropathy (nerve damage), blood flow, the ulcer size and depth, swabbing for infection and determining the cause (pressure, friction or minor trauma, which would not have been a problem in non-diabetic skin).

If there is infection in an ulcer we treat it surgically, removing the dead and infected tissue under local anaesthetic – otherwise bacteria will become trapped in the dead tissue and healing will not occur. We then grow the wound, treat the infection with specific antibiotics and wait for the wound to heal. Chronic ulcers that do not respond well to conservative treatment may be addressed using surgical pressure redistribution and tissue stimulation techniques. Education is important for all patients on foot inspection, appropriate footwear and regular follow up as it is important as much as getting the foot healed as prevention of new ulcers is essential. Sample of tissue is sent for culture and sensitivity in case of any suspicion of infection, thus using the right antibiotics for faster healing.

Comprehensive Foot Assessment
Detailed history, careful examination of ulcer – size and depth, drainage, odour, vascular assessment, neuropathy testing and swab for infection will be carried out during this appointment. Photographs taken to monitor progress, imaging only when we believe there is a deep tissue involvement.

Surgical Wound Management
Debridement and infection control is done in our minor surgery unit under local anaesthetic. All dead/infected tissue is carefully removed, samples sent for culture, prescribed specific antibiotics and offloading plans (special footwear or padding) are planned to ease the pressure. This can take 20 – 60 minutes depending on the severity of the ulcer.

Healing & Prevention Follow-Up
Follow up appointments are scheduled regularly to check the wound healing, dressing, circulation, and that the infection is resolving. After the ulcer has healed, we stress foot inspection every day, wearing the right footwear, and regular check-ups by a professional as prevention of new ulcers is vital to long term foot health.

Will the diabetic foot ulcer heal without treatment?
Even with good home care, most diabetic ulcers are difficult to heal without professional care due to the lack of nerve sensation and decreased blood flow caused by high blood sugar. Professional debridement, infection control and pressure management are key components.
What is the duration of the healing of a diabetic ulcer?
The healing time will vary based on the depth of the ulcer, blood flow, infection and blood sugar control. With appropriate care, a simple ulcer will heal in 4–6 weeks, and a deeper or infected ulcer may take 8–12 weeks or longer.
Why do we need debridement?
In an ulcer, dead tissue holds bacteria, starves the wound of blood supply and inhibits healing. The removal of this dead tissue by surgery allows living tissue to begin healing and antibiotics to be effective.
Neuropathy is what and how do it result in ulcers?
High blood sugar damages the nerves (neuropathy) and prevents you from feeling pressure or pain in your feet. A blister, cut or pressure point may not actually be felt until the ulcer is large.
What can I do to avoid developing foot ulcers due to diabetes?
Examine feet every day, maintaining clean and moisturized (between toes), cutting nails carefully, wearing shoes that are right for your feet, avoiding rubbing and pressure, and visiting a foot care specialist on a regular basis, while maintaining good blood sugar control.
Which if that ulcer returns, what will happen?
Recurrent ulcers are typically due to poor blood sugar control, footwear or offloading strategy. We evaluate the cause and modify your prevention plan, so that new ulcers do not develop.
If there is any evidence of a deep infection or gangrene of the ulcer, what happens?
If you develop a deep tissue infection or gangrene, you need to see a doctor right away and will need to have the infection aggressively debrided to save your foot. We take a careful approach to deciding the severity and work out a treatment plan that aims to heal and avoid amputation.
Can I return to my normal lifestyle after treatment for my ulcers?
Yes. Inspecting the feet daily, proper footwear, maintaining a controlled blood sugar level, regular professional foot examinations and preventive care are all important for the prevention of new ulcers. These are taught at each visit.
