Our Approach to Wound Care & Management
A wound that will not heal will likely have a problem, such as dead tissue which traps bacteria, poor blood flow, continued infection, or continued trauma from movement or pressure. As a starting point, we always assess the wound thoroughly, first by measuring the wound, then taking swabs to identify if there is an infection, checking blood flow to the surrounding tissue, checking for foreign material or debris, and determining the cause of the wound (infection, pressure, friction or poor circulation).
If a wound is infected we clean away all dead and infected material, take a specimen for culture (determine the type of infection causing the wound) and begin specific antibiotic therapy. When a wound does not improve with conservative treatment, we thoroughly clean the wound, optimize blood flow and may employ sophisticated wound management techniques to encourage the development of tissue. When the wound is acute, traumatic and we are able to assess the viability of the tissues well, we repair the deeper levels if necessary and proceed to primary closure when safe so that you heal quicker with minimal scarring. We teach all patients how to inspect wounds, how to dress wounds, and how to modify activities as it is what you do at home that will prevent the wound from healing or reopening. As the diagnosis is not always certain, we send cultures and histopathologies of tissues to enable us to use the correct treatment and therefore heal the patient quicker.
