About Case
The case concerns a 32-year-old man who came with a soft, painless lump which is slightly lateral to the midline of the back of the trunk. It had been progressively enlarging over some 3 years. No significant medical history and otherwise good health. His main concern was cosmetic and occasional discomfort in lying flat, and carrying a bag against the back. On examination the lump was non tender, soft, well defined and free-moving, which is typical of a benign lipoma.
No inflammation, fixity or skin changes were detected. A diagnosis of lipoma was made and the plan was to have it done under local anaesthetic.
Symptoms Encountered
At consultation the patient stated the following: A slowly growing soft swelling that has been there for 3 years on the back. Mild discomfort on pressure over swelling.
Challenges And Treatments
The lipoma presented clinically as a simple tumor but careful planning was necessary to assure a free and complete removal and minimal scarring. Positioning the patient slightly off-center back and also holding the surgical field carefully, was important at this point.
Challenges Encountered
The goal was to remove the lipoma entirely through the smallest possible incision while maintaining clean tissue planes throughout the procedure.
Brief Procedure
This was done under local anaesthetic in our surgery centre. Thorough skin preparation with sterile draping was done before a 3cm linear incision was made directly over the lipoma. The lipoma was carefully dissected off the surrounding subcutaneous tissue, keeping the capsule intact to ensure complete removal. Haemostasis was secured and closure was in layers. A superior wound eversion and tension distribution is achieved with the use of 4 vertical mattresses sutures that were particularly suitable for the back where skin tension is greater. At the end of the procedure a sterile dressing was applied.
Treatment Results
The specimen of lipoma excised was subsequently subjected to histopathological examination, which revealed a benign mature lipoma with no atypical features. The procedure was well tolerated without any intraoperative or postoperative complications. Excision with one single 3cm incision; Complete excision.
